4-H Outdoor Adventure Challenge Instructor Travel Reimbursement Form

Instructors giving leadership to 4-H OAC trainings may receive up to 50 cents per mile in travel reimbursement.  No lodging reimbursement is available. 

Please use this form when claiming mileage.  You may also add other expenses to this form that are incurred during this same travel time. 

Please note that MSU travel policy requires that each leg of your trip be accurately reflected for multiple destinations; i.e., from point A to B, B to C, C back to A).  A MapQuest printout is required before reimbursement can be processed.  Please provide such along with this completed travel reimbursement worksheet.  If one is not provided by the instructor, it will be handled at the State 4-H office and may delay processing.
