Clinic-Community Connections

Primary care practices need local infrastructure support in care coordination for patients to learn self-management strategies and health education. Effective clinic-community connections provide patients with help in changing unhealthy behaviors through educational program referrals and research.

The Rx for Health pad is a tool that healthcare providers and MSU Extension educators can use to increase patients’ awareness and use of MSU Extension's free and low-cost community-based health education programs. It can aid healthcare providers in building or maintaining a patient-centered medical home model of care (PCMH), as a PCMH requires that patients are referred to community and social services. Rx for Health is a simple and cost-effective way to achieve this.


How it works

Rx for Health pads are distributed by MSU Extension educators and personalized with their contact details. The referral pad itself can be used by anyone within the office of a healthcare provider and it does not require any data keeping. Any categories recommended for the patient can be checked with a pen or pencil. Then the tear-off sheet is given to the patient.
